Citi lab is pleased to announce the Loyalty Card program to facilitate patients. Card holders can avail discounts at Citi Lab head office.
You can purchase your loyaltycard for against a payment of Rs 500. Your personalized Loyalty Card shall be mailed to you at your address within fourteen (14) business days.
At the time of Loyalty Card request, please provide following information to Citi Lab representative in order to process your Loyalty Card.

The loyalty card will be valid for a period of 1 year from the date of issuance. You may renew it at any time against a payment of Rs. 300. In case of loss, please contact us for a replacement loyalty card.
